If you keep a food journal on your fast weight loss diet, you know exactly how many calories and what kind of nutrition you are taking to get your body in shape on a daily basis. For me personally, keeping a food journal has become second nature because I started watching my weight at a very young age. This is a good thing. I find this is a perfect way to document my performance, plus it offers accountability and something to show for all my effort.

Orthostatic influence on heart rate and blood pressure variability in trained persons with tetraplegia

To examine the orthostatic influence on heart rate and blood pressure variability in persons with tetraplegia playing wheelchair basketball, ten trained persons with tetraplegia, ten untrained persons with tetraplegia, and ten able-bodied participated in this study. Spectrum analysis of the ECG R-R interval and blood-pressure on a beat-by-beat basis during head-up tilt 60 degrees sitting were performed. The ratio of the high frequency to total frequency (HF/TF) in the R-R interval decreased from supine (0.5 +/- 0.2) to sitting (0.3 +/- 0.2), and handicap scooters, medical scooters the low frequency (LF) power in systolic blood pressure increased from 4.7 +/- 9.1 to 15.0 +/- 13.1 mmHg(2) only in the untrained persons with tetraplegia (P < 0.01). The decrease in the HF/TF ratio in the untrained persons with tetraplegia indicates attenuated parasympathetic activity to the orthostatic challenge and the similar increase in LF power indicate that parasympathetic activity was reduced and sympathetic activity increased only in these persons. These results suggest that training enhances cardiovascular stability in tetraplegic subjects

Aerobic capacity and peak power output of elite quadriplegic games players.

BACKGROUND: Participation in wheelchair sports such as tennis and rugby enables people with quadriplegia to compete both individually and as a team at the highest level. Both sports are dominated by frequent, intermittent, short term power demands superimposed on a background of aerobic activity. OBJECTIVE: To gain physiological profiles of highly trained British quadriplegic athletes, and to examine the relation between aerobic and sprint capacity. METHODS: Eight male quadriplegic athletes performed an arm crank exercise using an ergometer fitted with a Schoberer Rad Messtechnik (SRM) medicare power wheelchairs powermeter. The sprint test consisted of three maximum-effort sprints of five seconds duration against a resistance of 2%, 3%, and 4% of body mass. The highest power output obtained was recorded (PPO). Peak oxygen consumption (V(O2peak)), peak heart rate (HR(peak)), and maximal power output (PO(aer)) were determined. RESULTS: Mean PO(aer) was 67.7 (16.2) W, mean V(O2peak) was 0.96 (0.17) litres/min, and HR(peak) was 134 (19) beats/min for the group. There was high variability among subjects. power wheelchairs Peak power over the five second sprint for the group was 220 (62) W. There was a significant correlation between V(O2peak) (litres/min) and PO(aer) (W) (r = 0.74, p<0.05). CONCLUSIONS: These British quadriplegic athletes have relatively high aerobic fitness when compared with the available literature. Moreover, the anaerobic capacity of these athletes appeared to be relatively high compared with paraplegic participants.

Mechanical efficiency and propulsion technique after 7 weeks of low-intensity wheelchair training

BACKGROUND: To evaluate the effect of a 7-week low-intensity hand rim handicap scooters training on the submaximal metabolic cost, mechanical efficiency and propulsion technique in able-bodied participants. METHODS: Participants were randomly divided over an experimental group (n=14) and a control group (n=7). The experimental group received 7 weeks wheelchair training (3 week(-1), 70 min) at a low intensity (30% of the heart rate reserve), whereas the control group did not receive training. During pre- and post-tests, submaximal exercise was performed on a stationary wheelchair ergometer at fixed levels of power output. Mechanical efficiency, oxygen uptake, heart rate, timing parameters and stroke angles were measured. Video recordings were made to determine the stroke pattern. FINDINGS: Mechanical efficiency increased and metabolic cost decreased significantly in the experimental group compared to the control group. Push time increased and cycle frequency decreased as a result of training. The stroke angle increased in the experimental group during the training period. The experimental group preferred double-looping over propulsion, while the control group mainly used single-looping over propulsion patterns during the post-test. INTERPRETATION: A low-intensity, 7-week training protocol has a beneficial effect on the mechanical efficiency and metabolic cost of medical scooters propulsion in able-bodied participants. The improved mechanical efficiency seems to be the result of changes in propulsion technique that were found

Federally and privately funded research, foreign competition, and globalization appear to be factors that will drive bioengineering advances within the assistive technology (AT) industry. The seeds of bioengineering research are likely to contribute to improvements in universal design and the mainstreaming of products. The aims of AT have changed tremendously in the past 50 years. Most of the federal agencies supporting assistive and rehabilitative technology research and development did not exist 50 years ago. Indeed, the leading AT companies all were established within the past 50 years. Bioengineering has the potential to be to the 21 st century what electronic engineering was to the 20th century. Advances in power electronics, telecommunications, controls, sensors, and instrumentation have really only just begun to be applied for devices to assist people with SCI. Advancing technology for people with SCI represents a significant career and business opportunity for engineers who want to serve the public good in a meaningful and tangible way.

Review of the use of physical restraints and lap belts with wheelchair users

Wheelchair-related physical restraints, lap belts, and other alternatives are intended to provide safe and adequate seating and mobility for individuals using wheelchairs. Physical restraints and lap belts are also helpful for positioning people in their wheelchairs to reduce the risk of injury during wheelchair tips and falls. However, when used improperly or in ways other than intended, injury or even death can result. Although widely prescribed, little evidence is available to direct professionals on the appropriate use of these restraints and lap belts and for whom these restraints are indicated. The purpose of this study was to conduct a review of available literature from 1966-2006 to identify the risks and benefits associated with lap belts while seated in wheelchairs. Twenty-five studies that met the inclusion criteria were reviewed. Nine studies reported the frequency of asphyxial deaths caused by physical restraints, nine studies reported the long-term complication and indirect adverse effects of physical restraints and lap-belt use, and seven studies reported the benefits of physical restraints and lap belts with individuals using medicare power wheelchairs, handicap scooters. Despite the weak evidence, the results suggest a considerable number of deaths from asphyxia caused by the use of physical restraints occurred each year in the U.S. The majority of the deaths occurred in nursing homes, followed by hospitals, and then the home of the person. Most deaths occurred while persons were restrained in wheelchairs or beds. Based on that, caution needs to be exercised when using restraints or positioning belts. In addition, other seating and environment alternatives should be explored prior to using restraints or positioning belts, such as power wheelchair seating options. Positioning belts may reduce risk of falls from wheelchairs and should be given careful consideration, but caution should be exercised if the individual cannot open the latch independently. Also, the duration of use of the physical restraint should be limited. Therefore, several factors should be considered when devising a better quality of physical-restraint services provided by health care professionals. These efforts can lead to improved safety and quality of life for individuals who use wheelchairs

An experimental method for measuring the moment of inertia of an electric power wheelchair

This study describe an experiment measuring the moment of inertia of an electric medicare power wheelchairs (EPW) using a torsional pendulum method. Inertia of the wheelchair is an important factor for control, which is a key issue in wheelchair driving. The experimental test platform consisted of a bottom circular wood plate, an upper metal plate, and four ropes. Materials with known moments of inertia such as the metal disk and cylinder were used to test the accuracy of the system. The EPW used in the experiment was Invacare G3 Torque SP Storm Series. The measured result of the moment inertia of the wheelchair was 5.2280 kg.m(2) and handicap scooters the errors of the system are less than 10% even when the object is only 25lbs. The results are consistent when compared with other approximate methods. In addition, the experimental method could be used to measure the moment of inertia of manual wheelchairs and other irregular objects

Design and simulation of a pneumatic, stored-energy, hybrid orthosis for gait restoration

Loss of mobility due to lower limb paralysis is a common result of thoracic level spinal cord injury. Functional electrical stimulation (FES) can restore primitive gait in the vicinity of a wheelchair by using handicap scooters stimulation to generate muscle contractions. A new concept for FES-assisted gait is presented that combines electrical stimulation with an orthosis that contains a fluid power system to store and transfer energy during the gait cycle and medical scooters. The energy storage orthosis (ESO) can be driven through a complete gait cycle using only stimulation of the quadriceps muscles. The conceptual design of the ESO was completed and implemented in a dynamic simulation model and in a benchtop prototype for engineering measurements. No studies were conducted with human subjects. The results demonstrate the potential of the ESO concept for a feasible gait-assist system and the validity of the simulation model as a means for designing the system.

Study of the Independence IBOT 3000 Mobility System: an innovative power mobility device, during use in community environments

OBJECTIVE: To test the safe and effective use of a new mobility device, the Independence IBOT 3000 Mobility System, handicap scooters and medical scooters, by people with a disability. DESIGN: A prospective, balanced, open-label evaluation that used participants as their own controls. SETTING: Home and community environments. PARTICIPANTS: Twenty subjects who use a mobility device. INTERVENTIONS: Subjects used the test device for up to 2 weeks in their home and community environments. MAIN OUTCOME MEASURE: Safety was determined by comparing the number of reported adverse events, including falls, in the test device compared with their own device. Effectiveness was measured by comparing scores from the Community Drive Test in the test device versus their own chair. RESULTS: No adverse event requiring medical attention was reported for either device and a similar number of device falls were reported for each device. The scored driving test results found that the test device was shown to statistically improve (P <.001) the subjects independent functional mobility skills in a community environment. Ten of 20 subjects were able climb stairs independently; the remaining 10 subjects were able to climb stairs with the assistance of only 1 person. CONCLUSIONS: People who are properly assessed, and participate and successfully complete the training, will benefit through increased independence in community mobility activities.

Effects of graduated compression stockings on cardiovascular and metabolic responses to exercise and exercise recovery in persons with spinal cord injury.

OBJECTIVE: To investigate whether reporting blood redistribution by means of graduated elastic stockings affects exercise and postexercise responses in people with spinal cord injury (SCI). DESIGN: Crossover trial. SETTING: Physical medicine and rehabilitation department in France. PARTICIPANTS: Fourteen men with traumatic SCI, grouped according to their level of injury. INTERVENTIONS: Subjects performed 2 maximal medicare power wheelchairs exercise tests 1 week apart, in random order and under a counter-balanced design. One test was done with and the other without graduated elastic stockings (21 mmHg). MAIN OUTCOME MEASURES: Blood lactate, blood pressure, heart rate, maximal power output, and oxygen consumption (Vo2). RESULTS: Postexercise venous lactate concentration was reduced in SCI subjects with lesion levels below T6 while wearing graduated elastic stockings during both exercise and recovery (10.9+/-3.9 mmol/L vs 12.5+/-4.6 mmol/L, P<.05). There were no significant differences in submaximal and maximal values (heart rate, Vo2, power output) between subjects tested with and without graduated elastic stockings. CONCLUSIONS: Wearing elastic stockings affects postexercise responses by decreasing lactate concentration in well-trained, low-level paraplegic patients after a maximal exercise. The relatively low pressure generated by the stockings may not, however, influence the venous system enough to produce improved performance and cardiovascular responses

Power mobility device provision: understanding Medicare guidelines and advocating for clients

The Centers for Medicare and Medicaid (CMS) issued a new national coverage determination (NCD) for mobility assistive equipment (MAE) including power wheelchairs, handicap scooters in May 2005. CMS then issued a Final Rule in April 2006 that outlined significant changes required for documentation for prescribing MAE. Other insurance providers have since adopted and sometimes modified the NCD criteria and have begun to apply these criteria according to their own interpretations because some of the criteria are vague. In this report, we introduce a case example to show the components of the CMS NCD criteria, what was intended but poorly described in the language, how insurance providers may misinterpret or alter the criteria, and how clinicians can act as advocates.

Effects of respiratory resistance training with a concurrent flow device on wheelchair athletes.

BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, handicap scooters athletes. METHODS: Ten male wheelchair athletes (8 with spinal cord injuries, 1 with a neurological disorder, and 1 with postpolio syndrome), were matched by lesion level and/or track rating before random assignment to either a RRT group (n = 5) or a control group (CON, n = 5). The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.
