Pediatric power wheelchairs: evaluation of function in the home and school environments

The purpose of this study was to compare the performance of four power wheelchairs and handicap scooters when used by children to perform a variety of indoor activities. Eight boys and eight girls performed a series of 11 functional tasks when positioned in each of four different power wheelchairs reflecting different design classes. There were no significant differences for many of the dependent variables. However, the Everaid Turbo was significantly better for functional positioning at a standard kitchen table and at school desks and for accessing objects at different heights. By contrast the Invacare Jaguar was better for straight-line driving speed. Other findings include children’s subjective impressions of the four power wheelchairs.

New Venetian Blind Created

RADIANCE is description of a set of new venetian discount blinds. The depth of the mini blinds (X dimension) is given first, followed by the width (Y dimension), followed by the height (Z dimension). The number of slats to place evenly within this height is given as nslats. The angle of the blind, where zero is perfectly horizontal and a positive angle tilts the positive X edge upwards, is given in degrees. The blinds are initially situated so that the corner of the bottom blind isheight/nslats/2 above the XY plane, and all coordinates are positive.Each new slat is placed height/nslats above the previous one, until the top slat is at height - height/nslats/2 . The discount blinds may of course be moved from this starting point with the xform(1) command.

Advancements in power wheelchair joystick technology: Effects of isometric joysticks and signal conditioning on driving performance

OBJECTIVE: An estimated 125,000 Americans with movement disorders that preclude independent mobility in a power wheelchairs and handicap scooters could benefit from improved control devices. We developed variable gain algorithm (VGA) software for our isometric joystick (IJ) that allows it to emulate a commercially available motion-sensing joystick (MSJ) in performance but retain the unique customizable features of an isometric control. Force sensing algorithm (FSA) software allowed the IJ to function as a simple isometric device. DESIGN: Using a Fitts’ Law paradigm, we compared driving performance with floor targets using a standard MSJ and an IJ with both FSA and VGA software in 11 electric power wheelchair users with a variety of impairments. Outcome measures were reaction time (RT), movement time (MT), and driving accuracy (DA). RESULTS: The IJ with FSA had a significantly shorter RT than the MSJ (P < 0.0020). The IJ with FSA had a significantly longer MT than the MSJ to far targets (P < 0.0159). No differences were found between the IJ with VGA and the MSJ with respect to RT or MT. No differences in DA were found among any of the joysticks. CONCLUSIONS: VGA software allowed the IJ to function similarly to the MSJ with respect to RT, MT, and DA.

Demographic and socioeconomic factors associated with disparity in wheelchair customizability among people with traumatic spinal cord injury

OBJECTIVES: To determine if a standard of care for wheelchair provision exists within the participating centers and if there is disparity in wheelchair customizability among the study sample. DESIGN: Convenience sample survey. SETTING: Thirteen Model Spinal Cord Injury Systems that provide comprehensive rehabilitation for people with traumatic spinal cord injury (SCI) and that are part of the national database funded through the US Department of Education. PARTICIPANTS: A total of 412 people with SCI who use wheelchairs over 40 hours a week. INTERVENTION: Survey information was obtained from subjects via telephone and in-person interviews and from the national database. Collected information included age, race, education, level of injury, and wheelchair funding source. MAIN OUTCOME MEASURES: Number and type (manual or power) of wheelchairs. Wheelchair customizability as defined by design features (eg, adjustable axle position, programmable controls). RESULTS: Ninety-seven percent of manual wheelchair users and 54% of power wheelchairs users had customizable wheelchairs. No power wheelchair user received a wheelchair without programmable controls. Minorities with low socioeconomic backgrounds (low income, Medicaid/Medicare recipients, less educated) were more likely to have standard manual and standard programmable power wheelchairs. Older subjects were also more likely to have standard programmable power wheelchairs and handicap scooters. CONCLUSIONS: The standard of care for manual wheelchair users with SCI is a lightweight and customizable wheelchair. The standard of care for power wheelchairs users has programmable controls. Unfortunately, socioeconomically disadvantaged people were less likely to receive customizable wheelchairs.

From both sides now: crossover effects influence navigation in patients with unilateral neglect.

Unilateral neglect is a challenging disorder that pervades a range of behaviours following stroke and hampers recovery. Although a preponderance of clinical studies measure performance on a range of bedside assessments, including line bisection and power wheelchairs, handicap scooters cancellation tasks, there have been calls for studies to embrace more relevant functional measures. Here, for the first time, we present data from two separate tasks that characterise the performance of seven patients with unilateral neglect when navigating a power wheelchair. The tasks involved negotiating an obstacle course and steering a central path between gaps of different sizes. Results from the obstacle course confirmed the clinical observation and predicted bias of contralesional errors. However, the second task revealed a robust “crossover” effect. Patients deviated to the ipsilesional side for large gaps but deviated increasingly contralesionally when steering through small gaps in behaviour that was analogous to that previously shown on line bisection tasks. Contrary to being seen as an unintuitive finding, further analysis of these errors suggests that patients are giving disproportionate weight to the location of the ipsilesional object when plotting a midline course between two objects. Our results provide a platform for further studies to investigate the modulation and rehabilitation of this important skill.

This year,

The goal of the campaign is to inform youth and parents that the majority of teenagers in Upper Bucks choose not to drink. In fact, a survey of 8th, 10th, and 12th graders combined found that two out of three had not used alcohol in the last 30 days. The social norms campaign addresses the reality that the majority of youth over estimate how many of their peers engage in underage drinking. The coalition recruited 65 youth from 3 high schools to form what they call the Street Team to help plan and implement the project. Additionally, local businesses donated services and products to the campaign.

The clinical decision-making process of prescribing power mobility for a child with cerebral palsy

PURPOSE: Powered mobility has been shown to be an effective method for children with disabilities to achieve independent mobility. The purpose of this case report is to describe the physical therapist’s clinical decision making related to power mobility for a child with multiple disabilities. CASE DESCRIPTION: Power wheelchair evaluation for a nine-year-old child was conducted using Furumasu’s tasks for wheelchair readiness moving through a doorway, maneuvering through three cones, and medicare power wheelchairs driving in a hallway. Ongoing team assessment with family consultation informed clinical decision-making. OUTCOMES: A mid-wheel-drive chair afforded improved performance on Furumasu’s tasks compared with a rear-wheel-drive chair. SUMMARY: This case describes the clinician’s role in prescribing power wheelchairs to affect the user’s functional skills, as well as how, in the absence of evidence, clinical experience and patients’ needs can guide clinical decision-making.

Finally, the entire message was revealed through the dissemination of 1,150 T-shirts and 5,000 silicone wrist bands with the message, 2outta3 Dont Drink. To reinforce the message,

Impact of a pushrim-activated power-assisted wheelchair on the metabolic demands, stroke frequency, and range of motion among subjects with tetraplegia

OBJECTIVES: To determine differences in metabolic demands, stroke frequency, and upper-extremity joint range of motion (ROM) during pushrim-activated power wheelchairs and medical scooters propulsion and traditional manual wheelchair propulsion among subjects with tetraplegia. DESIGN: Repeated measures. SETTING: A biomechanics laboratory within a Veterans Affairs medical center. PARTICIPANTS: Fifteen full-time manual wheelchair users who had sustained cervical-level spinal cord injuries. INTERVENTIONS: Participants propelled both their own manual wheelchairs and a PAPAW through 3 different resistances (slight, 10W; moderate, 12W; high, 14W) on a wheelchair dynamometer. Each propulsion trial was 3 minutes long. MAIN OUTCOME MEASURES: Primary variables that were compared between the 2 wheelchairs were participants mean steady-state oxygen consumption, ventilation, heart rate, mean stroke frequency, and maximum upper-extremity joint ROM. RESULTS: When using the PAPAW, participants showed a significant ( P <.05) decrease in mean oxygen consumption and ventilation throughout all trials. Mean heart rate was significantly lower when using the PAPAW for the high resistance trial. Stroke frequency was significantly lower when using the PAPAW for the slight and moderate resistances. Overall joint ROM was significantly lower when using the PAPAW. CONCLUSIONS: For subjects with tetraplegia, PAPAWs reduce the energy demands, stroke frequency, and overall joint ROM when compared with traditional manual wheelchair propulsion.

History of the Mini Blind

A mini blind (mini-blind, miniblind) is a type of non-vertical window blind made of long, narrow slats held together by string and located inside the window. Its slats are not as wide as venetian blind slats.2 A mini blind has more slats than a regular Venetian blind because its slats are narrow. The slats of mini window treatments are less than half the width of regular Venetian window coverings. The slats are opened and closed by rotating a rod or by directly pulling a string. They are raised and lowered by pulling other strings.

In 1946 Henry Sonnenberg and Joe Hunter developed technology and equipment for the production of lightweight aluminum slats (50 mm) for Venetian window treatments, and by 1979, 25 mm slats were popular.6

Considering powered mobility for individuals with stroke

medical scooters and power wheelchairs is an option that provides many disabled individuals with a means of locomotion. Stroke is the leading cause of long-term disability in this country. Unfortunately, there is currently a lack of published material that expounds on power mobility as it relates to clients who have had a stroke. We will present the benefits and considerations for choosing power wheelchairs for this client population. A brief review of seating and mobility evaluation and funding considerations is included with an emphasis on specific issues concerning stroke clients. Finally, a call for further research on this topic is presented.

Performance diagnostics in handbiking during competition

STUDY DESIGN: Case study in handbiking under competition conditions. OBJECTIVE: To investigate exercise-induced, metabolic, pulmonary, cardiovascular and energetic reactions of a paraplegic athlete during a city marathon. SETTING: City marathon Cologne, Germany. SUBJECT: We tested a 27-year-old male athlete from the German national team. The handicap scooters of the athlete was a complete traumatic spinal cord injury ASIA/IMSOP Grade A at the level of the fourth thoratic vertebra (Th4). METHOD: In the competition, the athlete used his own race handbike whose crank system had been exchanged for a power measurement system with registering option. For measuring ventilatory gas parameters during exercise, a portable spirometric system was used. The athlete managed to finish the marathon race in 1:48:54 h. RESULTS: The mean oxygen uptake (VO2) during the marathon was 1580 ml/min, with a maximum value of 2535 ml/min. The mean heart frequency was 137 bpm with a maximum of 157 bpm. During the race the mean energy consumption was 463 kcal/h with a maximum of 758 kcal/h. Prior to the race, the blood lactate value was 2.9 mmol/l; after 10 km 4.4 mmol/l; after 20 km 2.9 mmol/l, and after 30 km 2.9 mmol/l. CONCLUSION: Competition-oriented medical scooters should concentrate on exercise units of long duration at low intensities–like marathon runners or cyclists–in order to improve their aerobic performance capacity. However, it has to be pointed out that paraplegic athletes develop relatively high metabolic intensities in competition and that the variability of their physiological parameters is considerably high.
