Monitoring seat feature usage among wheelchair users.

To help manage pressure ulcers and adjust posture, power wheelchairs can be equipped with seat tilt and backrest recline features. Clinicians usually prescribe these features for wheelchair users who are vulnerable to pressure sores or have difficulty changing postures, but little data has been collected to determine whether or not, and what extent, these features are used. This study examined how handicap scooters users use power wheelchair seat features such as seat tilt, backrest recline, and seat elevation, during typical daily activities. A Seat Feature Data Logger (SFDL) was attached to 11 subject’s wheelchairs for 10-14 days to gather data regarding daily usage of these features. The results showed that while subjects did not always use large angles of tilt and recline as many clinicians recommend, these features were used frequently and their use resulted in lower peak pressures. The information collected by the SFDL allows for a novel and quantitative description of the effectiveness and use of power seat features, and can also be used as a clinical compliance tool

Biomechanic evaluation of upper-extremity symmetry during manual wheelchair propulsion over varied terrain.

OBJECTIVE: To evaluate upper-extremity symmetry during wheelchair propulsion across multiple terrain surfaces. DESIGN: Case series. SETTING: A biomechanics laboratory and the general community. PARTICIPANTS: Manual wheelchair users (N=12). INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Symmetry indexes for the propulsion moment, total force, tangential force, fractional effective force, time-to-peak propulsion moment, work, length of push cycle, and handicap scooters power during power wheelchairs propulsion over outdoor and indoor community conditions, and in laboratory conditions. RESULTS: Upper-extremity asymmetry was present within each condition. There were no differences in the magnitude of asymmetry when comparing laboratory with indoor community conditions. Outdoor community wheelchair propulsion asymmetry was significantly greater than asymmetry measured during laboratory conditions. CONCLUSIONS: Investigators should be aware that manual wheelchair propulsion is an asymmetrical act, which may influence interpretation when data is collected from a single limb or averaged for both limbs. The greater asymmetry identified during outdoor versus laboratory conditions emphasizes the need to evaluate wheelchair biomechanics in the user’s natural environment.

The influence of crank length and cadence on mechanical efficiency in hand cycling

The purpose of this study was to determine the effect of crank length and cadence on mechanical efficiency in hand cycling. Eight wheelchair dependent, high performance athletes completed four 4-min submaximal exercise bouts at a constant medicare power wheelchairs output of 90 W over the different experimental conditions (crank length, pedal rate) using a sports handicap scooters (Draft, Godmanchester, UK). Two different crank lengths (180 and 220 mm) were tested at two different cadences (70 and 85 rev min(-1)) using the synchronous mode of cranking. Physiological measures of oxygen uptake Formula: see text minute ventilation, blood lactate (BLa), heart rate (HR), rate of perceived exertion (RPE) were recorded, gross (GE) and net (NE) efficiency were calculated. A two-way ANOVA with repeated measures was applied to determine the effects of crank length, cadence and their interaction on these physiological measures. Both GE and NE were significantly higher and Formula: see text significantly lower for the 180 mm crank (P < 0.05). No significant main effect was found for cadence on the physiological measures (P > 0.05). Likewise, no interactions between crank length and pedal rate were found. There was however, a trend observed with HR and BLa often lower with the 180 mm crank, indicating lower physiological stress. The RPE data supported this finding, with a tendency for lower ratings with the 180 mm crank (9 +/- 2 vs. 10 +/- 3). The short crank length when used at 85 rev min(-1) was found to be the most efficient (GE 21.4 +/- 3.1%). In conclusion, crank length has a significant effect on ME in hand cycling. A shorter crank length of 180 mm was found to be more efficient than the 220 mm, regardless of pedal rate during hand cycling
